on the form it says sponsors name what is that is suposed to mean Confused please help

don't worry about that. that's wher a demolay will sign it. just fill everything else out

Like Tennesseebrother said, don't worry about it. Just fill out the petition, and show it to a DeMolay at your closest chapter, he'll happily sign it for you.
